You can use the secure on-line registration form or enter your information on the PDF registration form.

Please submit your registration according to your choice of payment as stated below: 

·            If paying by check: Fax the completed registration form and copy of completed check to (916) 313-3298, followed by hard copies in the mail to our “remit to only address”—File No. 73038, P.O. Box 60000, San Francisco, CA  94160. 

·            If paying with a purchase order: Fax the completed registration form, along with purchase order, for quick processing, or mail to 1121 L Street, Suite 1060, Sacramento, CA  95814. Online registrations are also accepted with a purchase order number notated on the registration form.  

·            If paying by credit card: Fax the completed registration form, along with the credit card number to (916) 313-3298, or mail to 1121 L Street, Suite 1060, Sacramento, CA 95814. You can also submit your registration online and include your credit card number and expiration date on the registration form.
